We are open for registrations for people who have newly moved to Delft within our practice area. If you already have a GP in Delft, it is not possible to register with us.
The area we cover is within a radius of about 1 km around the practice; we do not follow the exact postal code boundaries.
Please call the practice first to check if your address falls within our area.

If you live in our area, you can come to the practice with the fully completed registration forms, ID, and insurance card; this applies to all family members.
You can come to the desk between 8:00 and 12:30, and between 13:30 and 16:30. Registration by email is no longer possible; you need to come to the practice in person.
